The Need

More than 185,000 people in Orange County are uninsured and have no access to ongoing care due to insurance status and cost barriers. Grace Medical Home is here to change that.

Grace reflects the heart of Christ by offering the highest level of health care to the low-income, uninsured in Orange County. To ensure every patient has access to the care and support to address their unique health care needs, primary care providers work in tandem with mental health counselors, dieticians, specialty care physicians, dentists, spiritual care volunteers and others in a comprehensive care model.

Grace also addresses the social determinants of health by connecting patients with community resources they need to achieve optimal health outcomes. Since its inception in 2010, Grace has served more than 5,000 patients and reduced patients’ ER visits totaling more than $2.4 million since 2015.

A Medical Home

A medical home is not a building, house or hospital, but rather an approach to providing continuous, comprehensive primary care. Think of a medical home as a hub or home base where a patient’s medical history is known and their medical care is coordinated.  Patients have an ongoing and personal relationship with their physician and a place to turn for their healthcare needs.

History

Grace Medical Home is the mutual dream of multiple dedicated leaders in Central Florida. Its foundation began with the shared belief that as recipients of God’s love and mercy we are called by Jesus to serve our neighbors.

Years ago, local physicians participated in a medical mission trip to the Dominican Republic. The mission was to help the Dominicans turn a children’s sick clinic into a comprehensive medical home where, in addition to acute sick visits, there are well care check-ups, health education and adequate medical records. Returning home, they saw the same need to provide high quality and continuous healthcare to the underserved right here in Orlando.

As this idea and vision spread, other healthcare professionals with similar hopes and dreams stepped forward to join the effort. Some were already providing healthcare services for the underserved of Central Florida and were witnessing first-hand the need to help restore the physical health of their patients. They, too, were envisioning a place where their current patients could receive quality health education and continuity of care.

Business and community leaders joined alongside these physicians to access the healthcare needs of Orlando and began to raise funding for operations. Together, relying fully on the perfect plan of Jesus Christ, a team was formed to establish Grace Medical Home.

Grace Medical Home received its 501(c)(3) status in January 2008. The countless hours of dedicated volunteers have brought us to where we are today. Hundreds of volunteers of all ages and stages of life continue to donate their time, talents and treasure to the vision of Grace.
